Communication that works
Closed loop: say it, hear it read back, confirm.
Speak up: name the concern, the reason and what you want ("Captain, we are 20 kt fast at 1,000 ft, the approach is not stable: go around"). If the first call is ignored, call again, then act as the operator's procedures say.
Brief the threats, not only the routine.
